VOLUNTEER COACH FORMS & CLEARANCES

1. Complete the following MLBA VOLUNTEER FORMS:
   1. MLBA Volunteer Coach Application Form
   2. MLBA Volunteer Coach Agreement Form
2. Apply for the following PA STATE PROTECTION LAWS AND CLEARANCES:
   1. ACT 34 - PA Criminal History Check
   2. ACT 151 - PA Child Abuse History Check
   3. ACT 114 - FBI Criminal History Report
   *FBI Criminal History Report
   If you have lived in Pennsylvania for the past 10 years, you must affirm your 10-year PA residency by signing a Volunteer Affirmation Form
   The FBI Criminal History Background Check is NOT required if a volunteer has been a resident of Pennsylvania for the past 10 years and swears or affirms
   that they have not been convicted of a prohibited offense listed in existing law or a similar offense in another jurisdiction.
   
   
3. Obtain a MT. LEBANON SCHOOL DISTRICT VOLUNTEER CARD verifying all PA  state protection laws and clearances. This card can be obtained in person by showing any Mt. Lebanon School District Secretary—at any of our schools—your PA state protection laws and clearances certification documents.
   
   
4. SUBMIT COPIES proving your clearances certifications noted above to the MLBA Board via email. 
   
   
5. Maintain an ACTIVE USER ACCOUNT on the MLBA website.
   
   
6. Log in to the website and REGISTER AS A VOLUNTEER for this year's season.
   
   
7. ACTIVELY COMMUNICATE WITH YOUR TEAM about practice and game schedules.  Team pages are located under the TEAM CENTRAL section of the website (accessible only when logged-in to your account). There’s also a new TeamManager mobile app offered by Dick’s Sporting Goods.
